The complex process and piles of paperwork needed to file a Veterans Disability claim can create an overwhelming experience if you are trying to apply for VA benefits. There are many forms and documents required to properly file a Veteran’s Disability claim, but do no fret; here is a comprehensive list that can help ease some of the confusion.
First and foremost is the VA Form 21-526EZ, or Veteran’s Application for Compensation and/or Pension, which can be downloaded from the Department of Veteran Affairs website. Once completed, you must submit (or mail) this document to your closest Veteran Affairs regional office. Additionally, you can fill the form out online with the Veteran’s Online Application (VONAPP). You can also call the Veteran’s Affairs National office at 1-800-827-1000 to have the VA Form 21-526EZ mailed to your home.
When applying and filling out the VA Form 21-526EZ, you will need access to:
- Discharge or Separation Papers for all periods of Service ( DD214 – Military Service Record Request); if you do not have these, you can visit the National Archives website for help.
- Medical Evidence, which includes your doctor and hospital reports regarding your disability.
- Dependency Records, which include marriage and children’s birth certificates.
Don’t forget, some eligible service members may be able to receive benefits sooner if the application is completed before discharge from the service through Benefits Delivery at Discharge (BDD).
If you want to file for Dependency and Indemnity Compensations (DIC) you will need the VA Form 21-534EZ. This form is for survivors of military service members who died in the line of duty, and survivors of Veterans whose death resulted from a service-related injury or disease, who wish to receive VA benefits.
